Changes to funding will make it in the interests of hospitals for patients to be discharged only when ready and safe for them to do so. With costs of up to £43m for local NHS Trusts to treat patients readmitted to hospital within 30 days of discharge, an AHSN-sponsored seminar, explored how health and social care teams can work together to achieve effective discharge and effective reablement to improve outcomes for patients.
